Metabolic Healing, LLC Health Consulting Contract
By electronically signing this contract, you hereby agree to the following terms and conditions.
You agree to pay $800 per month for 6 consecutive months, in exchange for 6 monthly consulting sessions executed by Michael McEvoy. You also receive 30 minutes of additional email communication with Michael each month during the duration of the program.
You understand that any health, nutritional, dietary or supplemental advice, opinions or recommendations given during your consultation sessions or via email correspondence are solely for educational purposes and should not be used to replace medical care or medical services.
All consulting package purchases are final. There will be no refunds issued after purchase is completed. Once you agree to the terms in this contract, Metabolic Healing, LLC is obligated to fulfilling the contractually agreed upon consulting services.
Six Month Health Consulting Programs
The 6-month Metabolic Healing Health Consulting Program includes the following:
- 6 months of consulting services, consisting of 1 consulting calls or video conferences per month.
- Consultations are performed through any of these listed methods, or any combination of the following: phone, internet phone service, skype, video conferencing, or email
Logistics of Scheduling
Consultations are scheduled through the Metabolic Healing system. Your consultant will make every effort to schedule consultations with you. In order for scheduling to occur, you must provide a valid email address, phone number, skype, zoom, or video conferencing ID and acknowledge receipt of email. If your email system blocks, rejects or sends Metabolic Healing consultant emails to your junk or spam folders, this may delay booking of your consultations. Metabolic Healing consultants will not be held liable if your email system does not adequately receive consultant emails.
In the instance where you must reschedule a consultation, you agree to give a minimum of 24 hours notification to the consultant. Notification for a rescheduling request less than 24 hours prior to the start of a consultation will result in a missed consultation, and cannot be rescheduled. If your consultant must reschedule, you will be given 24 hours of advanced notification of the cancellation, except in the instance of an emergency.
In the instance where you have outstanding consultations yet to be performed after the end of the 6-month billing cycle, you will be given a 45-day grace period to complete your consultations. If your consultations are not finalized after this 45-day grace period, you will be billed at an hourly rate of $350 per hour to complete any outstanding consultations.
If your Metabolic Healing consultant is no longer able to perform consultations with you for any reason, you will be assigned to work with a different Metabolic Healing consultant, in order to successfully complete your consulting program.
Logging Time of Consultations
Your Metabolic Healing consultant maintains a log of all consultations performed.
