Metabolic Healing, LLC Clinical Mentorship Contract
Terms of Service
By signing this contract you agree to the terms of service of the Metabolic Healing 3-month mentorship program. You agree to your full participation in the Metabolic Healing Clinical Mentorship program, led by Michael McEvoy and Shawn Bean.
You agree to pay $3,000 per month for 3 consecutive months to Metabolic Healing, LLC (a California Limited Liability Corporation) in exchange for 3 months of mentorship consulting services executed by Michael McEvoy and Shawn Bean. You understand that payment is paid to Metabolic Healing, LLC in 3 monthly payments, and is billed through an automated payment gateway through the Metabolic Healing site. You understand that once the first payment is completed, there is no refund of the program, nor cancellation of the program.
Monthly Mentorship Logistics
The purpose of the Metabolic Healing, 3-month Clinical Mentorship program is for educational purposes only. Metabolic Healing, LLC, Shawn Bean and Michael McEvoy will not diagnose or treat any person, including your clients or case studies. Any information, clinical details or therapeutic protocol discussions should be taken solely as educational material. Metabolic Healing LLC, Michael McEvoy and Shawn Bean are not liable or responsible in any way for your treatments, therapies, advice, consulting or coaching for your client’s or patients health.
You agree to your full participation in the program. This includes 2 one-on-one calls each month with Michael McEvoy and Shawn Bean, and 1 group call. Calls are conducted through any of the following: Zoom, Skype, Phone, internet phone service.
All mentorship calls are recorded and you understand that you have full access to these recordings for up to 90 days following the completion of your mentorship program. All recorded calls are for your personal, educational use only. Metabolic Healing, LLC retains the rights and ownership to all recorded mentorship calls. Any unauthorized use, publication, distribution, or commercial sales of mentorship calls is strictly forbidden and prohibited. As part of this agreement, Metabolic Healing, LLC may use, distribute, sell or retain recorded mentorship calls to be used at a later date.
Standard 24-Hour Cancellation Policy
Students enrolled in the mentorship program are expected to participate in every mentorship session. If you cancel mentorship sessions less than 24 hours prior to the scheduled call, you will not be allowed to reschedule.
Missed Appointments
If for whatever reason, you are unable to complete all scheduled one-on-one mentorship sessions during the 3-month program, you have a grace period of 21 days after the end of the 3-month program to complete any remaining mentorship sessions.
Anonymization of Personal Health information (PHI)
Metabolic Healing, LLC, Michael McEvoy and Shawn Bean will only teach mentorship lessons for client’s or patient’s whose PHI (personal health information) has been appropriately anonymized. This must include:
- Removal of all identifiers: Name, email address, home address, phone numbers, social security numbers, or ANY other information that is linked or traceable to a person.
- You agree to anonymize all client/patient data that is used in the mentorship program. This must be done prior to any review with Michael McEvoy or Shawn Bean.
- Lab testing, and other clinical data will not be analyzed or included in mentorship lessons unless it has been fully and appropriately anonymized.
